Privacy Policy 

   Petworth Therapy - Laura Woodham will hold the following information 

I am registered with the ICO (Information Commissioners Office) which requires that I tell you what data I am collecting from you and what I intend to do with it.

 

What data do I collect and why?

I collect relevant personal information so that I can provide you with a professional therapeutic service and meet legal requirements.

The records I keep include your name, address, email and phone numbers – so that I can contact you regarding our sessions. I keep a record of your attendance at sessions, and your payments. Sometimes I keep brief notes on our sessions. These are anonymised.

I keep a record of your GP’s contact details, and occasionally other relevant health professionals contact details in case you ever have a medical emergency. I keep a record of any medication you might be taking. I would only contact your doctor (s) with your permission.

 

Who can see this information?

I make every effort to keep your information confidential. All my records, both on paper and those held electronically, are stored securely.

To ensure good professional practice, I consult with colleagues about my work. However, I take good care to protect your confidentiality by disguising personal details and making sure that such presentations are anonymised.

No personal information is passed on except in the very rare situation where you might be at risk to yourself or others or there are safeguarding concerns. In this event, there may be a necessity to make contact with someone such as your GP.

Your rights

You do have the right to see any information I have about you. You may also ask for any information to be deleted once your therapy has finished.

How long will I hold your information?

I will keep relevant information for tax purposes for the time required by HMRC (7 years).

I will keep other information about you for no more than 7 years from when your therapy ends.
